    Human error is to blame for the death of two preemies who received adult doses of blood thinner. Two babies born at 25 and 26 week's gestation died Saturday night. Four other infants in the Newborn Intensive Care Unit (NICU) at Methodist Hospital also received the doses of Heparin. Three were in serious condition and one may require surgery.
